Purpose: Regorafenib is an oral multikinase inhibitor approved for the therapy of previously treated metastatic colorectal carcinoma (mCRC). The aim of the present study was to analyze the outcomes of treatment with regorafenib in real-world clinical practice based on data from a national registry. Methods: The CORECT registry, the Czech non-interventional database of patients with mCRC treated with targeted agents, searched for patients with metastatic CRC treated with regorafenib. In total, 555 evaluable patients were identified. Results: The median age at diagnosis was 61.7 years. All patients had disease progression on or after previous systemic treatment. Most patients were treated with an initial dose of 160 mg daily (n = 463; 83.6%). The median duration of treatment was 2.7 months (range 0.023.4 months). By the data cut-off date, 472 patients (85%) had completed treatment with regorafenib and were evaluable for treatment response evaluation. Partial response was reported in 13 patients (2.8%) and disease stabilization in 130 patients (27.5%). Median progression-free survival (PFS) and overall survival (OS) were 3.5 months (95% confidence interval [CI] 3.2-3.7 months) and 9.3 months (95% CI 8.3-10.3 months), respectively. The 6-month OS rate was 67.7% (95% CI 63.4-72.1%). Multivariable analysis showed that female gender, longer interval from diagnosis of metastatic disease, MO stage at diagnosis, and Eastern Cooperative Oncology Group performance status (ECOG PS) 0 were associated with longer PFS, while higher body-mass index (BMI), longer interval from diagnosis of metastatic disease, and ECOG PS of 0 were associated with longer OS. Conclusion: OS of patients treated with regorafenib in the real-world clinical practice in this cohort exceeded that reported in randomized trials. Regorafenib is a safe and active treatment option for a subgroup of patients with mCRC who are progressing after other systemic therapies and maintain good performance status.

Ampelocalamus scandens is native to Guizhou Province, China, and grows at an altitude of 260-320 m. It can be used as a raw material for weaving and papermaking. In the current study, the complete chloroplast (cp) genome of A. scandens was sequenced and is reported for the first time. The complete cp sequence was 139,504 bp, include large single-copy (LSC), small single-copy (SSC), and a pair of invert repeats (IR) region of 83,103 bp, 12,813 bp, and 21,793 bp, respectively. Besides, the plastid genome comprised a total of 132 genes, including protein-coding, tRNA, and rRNA genes as 85, 39, and 8 genes, respectively. Phylogenetic analysis based on 28 cp genomes reveals that A. scandens is closely associated with Ampelocalamus melicoideus in Arundinarodae.

Here in, the effects of FeSiAl particle size on the dielectric and microwave absorption properties of FeSiAl/Al2O3 composites were studied. FeSiAl/Al2O3 composites containing 18-25 mu m, 25-48 mu m, and 48-75 mu m FeSiAl particles were prepared by hot-pressed sintering based on uniformly mixed FeSiAl and Al2O3 powders. Results show that the real permittivity and the imaginary permittivity are significantly promoted with increasing FeSiAl particle size, which is ascribed to the enhanced interfacial polarization and conductance loss. In addition, the favorable matching impedance and suitable attenuation coefficient enabled the composite containing 25-48 mu m FeSiAl powder to show a minimum reflection loss of -34.4 dB at 11.7 GHz and an effective absorption bandwidth (<-10 dB) of 1.4 GHz in 11.0-12.4 GHz, when the thickness is 1.1 mm. By adjusting the thickness to 1.4 mm, the effective absorption bandwidth of the composite reaches a maximum value of 2.0 GHz in the 8.3-10.3 GHz range, indicating tunable, strong, and highly efficient microwave absorption performance. Mango waste is one of the most promising sources of renewable energy, especially as this waste represents 40% of the weight of mango fruit and contains a large amount of fat and cellulose that can contribute to converting it into energy products using pyrolysis and gasification process. Within this context, this research aims to investigate pyrolysis and gasification kinetic behavior of mango seed shells (MSS) using TG-FTIR-GCeMS system. The experiments were started by analyzing the composition of different types of Egyptian MSS, then their pyrolysis characteristics and chemical decomposition in N-2 and CO2 atmospheres using TG-FTIR system upto 900 degrees C at heating rates in the range 5-30 degrees C/min were studied. The GC/MS system was employed to determine the formulated volatile products at the maximum decomposition temperatures (343-346 degrees C for N-2 and 334-340 degrees C for CO2). Afterwards, the model-free/model-fitting methods, including KissingereAkahiraeSunose, FlynneWalleOzawa, and Friedman, and Distributed Activation Energy Model (DAEM) were used to estimate the kinetic parameters of pyrolysis of MSS in both atmospheres. Finally, chars derived from pyrolysis were exposed to CO2 gasification process, followed by studying of their kinetic behavior in the modified random pore model (MRPM). The results showed that the decomposed MSS were saturated with a huge amount of volatile products, particularly Carbon dioxide and Ethylene oxide (99.27% in CO2 and 20.77% in N-2), while Acetic acid, Propanone, Hexasiloxane, Glycidol, Ethanedial, Ethylene oxide, Formic acid, etc. were the main compounds in case of N-2. Meanwhile, the studies of kinetics of pyrolysis showed that the average activation energies were estimated in the range of 231-262 kJ/mol (N-2) and 259-333 kJ/mol (CO2). Based on that, pyrolysis and gasification can be adapted as promising technologies to valorize MSS and utilize them as a new sustainable source for renewable energy. (C) 2021 Elsevier Ltd. Multinuclear Ag clusters sandwiched by Pt complex units were synthesized and characterized by single crystal X-ray diffraction and NMR studies. The sandwich-shaped multinuclear Ag complexes showed two different types of fluxional behavior in solution: rapid slippage of Pt complex units on the Ag-3 core and a reversible demetalation-metalation reaction by the treatment with Cl anion and Ag ion, respectively. The Ag-2 complex obtained by demetalation reaction from the Ag-3 complex displayed U to Z isomerization. These multinuclear Ag complexes showed strong photoluminescence whose properties depended on the existence of Pt -> Ag dative bonds. The Ag-3 complex, identified to be chiral-at-cluster, was optically resolved by the formation of a diastereomeric salt with a chiral anion. The enantiomers show circular dichroism (CD) and circularly polarized luminescence (CPL) properties which is unprecedented for compounds based on a chiral sandwich structure. Theoretical calculations allow to understand their structural features and photophysical properties.

Background: The high prevalence of physician burnout, particularly in emergency medicine, has garnered national attention in recent years. Objective means of measuring stress while at work can facilitate research into stress reduction interventions, and wearable photoplethysmography (PPG) technology has been proposed as a potential solution. However, the use of low-burden wearable biosensors to study training and clinical practice among emergency physicians (EP) remains untested. Objective: This pilot study aimed to (1) determine the feasibility of recording on-shift photoplethysmographic data from EP, (2) assess the quality of these data, and (3) calculate standard pulse rate variability (PRV) metrics from the acquired dataset and examine patterns in these variables over the course of an academic year. Methods: A total of 21 EP wore PPG biosensors on their wrists during clinical work in the emergency department during a 9-hour shift. Recordings were collected during the first quarter of the academic year, then again during the fourth quarter of the same year for comparison. The overall rate of usable data collection per time was computed. Standard pulse rate (PR) and PRV metrics from these two time points were calculated and entered into Student t tests. Results: More than 400 hours of data were entered into these analyses. Interpretable data were captured during 8.54% of the total recording time overall. In the fourth quarter of the academic year compared with the first quarter, there was no significant difference in median PR (75.8 vs 76.8; P=.57), mean R-R interval (0.81 vs 0.80; P=.32), SD of R-R interval (0.11 vs 0.11; P=.93), root mean square of successive difference of R-R interval (0.81 vs 0.80; P=.96), low-frequency power (3.5×103 vs 3.4×103; P=.79), high-frequency power (8.5×103 vs 8.3×103; P=.91), or low-frequency to high-frequency ratio (0.42 vs 0.41; P=.43), respectively. Power estimates for each of these tests exceeded .90. A secondary analysis of the resident-only subgroup similarly showed no significant differences over time, despite power estimates greater than .80. Conclusions: Although the use of PPG biosensors to record real-time physiological data from EP while providing clinical care seems operationally feasible, this study fails to support the notion that such an approach can efficiently provide reliable estimates of metrics of interest. No significant differences in PR or PRV metrics were found at the end of the year compared with the beginning. Although these methods may offer useful applications to other domains, it may currently have limited utility in the contexts of physician training and wellness.
