Background: The high prevalence of physician burnout, particularly in emergency medicine, has garnered national attention in recent years. Objective means of measuring stress while at work can facilitate research into stress reduction interventions, and wearable photoplethysmography (PPG) technology has been proposed as a potential solution. However, the use of low-burden wearable biosensors to study training and clinical practice among emergency physicians (EP) remains untested. Objective: This pilot study aimed to (1) determine the feasibility of recording on-shift photoplethysmographic data from EP, (2) assess the quality of these data, and (3) calculate standard pulse rate variability (PRV) metrics from the acquired dataset and examine patterns in these variables over the course of an academic year. Methods: A total of 21 EP wore PPG biosensors on their wrists during clinical work in the emergency department during a 9-hour shift. Recordings were collected during the first quarter of the academic year, then again during the fourth quarter of the same year for comparison. The overall rate of usable data collection per time was computed. Standard pulse rate (PR) and PRV metrics from these two time points were calculated and entered into Student t tests. Results: More than 400 hours of data were entered into these analyses. Interpretable data were captured during 8.54% of the total recording time overall. In the fourth quarter of the academic year compared with the first quarter, there was no significant difference in median PR (75.8 vs 76.8; P=.57), mean R-R interval (0.81 vs 0.80; P=.32), SD of R-R interval (0.11 vs 0.11; P=.93), root mean square of successive difference of R-R interval (0.81 vs 0.80; P=.96), low-frequency power (3.5×103 vs 3.4×103; P=.79), high-frequency power (8.5×103 vs 8.3×103; P=.91), or low-frequency to high-frequency ratio (0.42 vs 0.41; P=.43), respectively. Power estimates for each of these tests exceeded .90. A secondary analysis of the resident-only subgroup similarly showed no significant differences over time, despite power estimates greater than .80. Conclusions: Although the use of PPG biosensors to record real-time physiological data from EP while providing clinical care seems operationally feasible, this study fails to support the notion that such an approach can efficiently provide reliable estimates of metrics of interest. No significant differences in PR or PRV metrics were found at the end of the year compared with the beginning. Although these methods may offer useful applications to other domains, it may currently have limited utility in the contexts of physician training and wellness.

The direct reaction of a series of substituted (1H-pyrazol-1-yl)pyridazine (L-I: 6-(1H-pyrazolyl)pyridazine; L-II: 3-chloro-6-(1H-pyrazole-1-yl)-pyridazine; L-III: 6-(1H-3,5-dimethylpyrazolyl)pyridazine-3-carboxylic acid; L-IV: 3,6-bis-N-pyrazolyl-pyridazine; and L-V: 3,6-bis-N-3-methylpyrazolyl-pyridazine) with the bromotricarbonyl(tetrahydrofuran)rhenium(I) dimer leads to the monometallic complexes [(L-X)Re(CO)(3)Br] (I-V), which displays a nonregular octahedral geometry around the Re-I center and a fac-isomerism for the carbonyl groups, whereas pyridazine and pyrazolyl rings remain highly coplanar after coordination to rhenium. Cyclic voltammetry shows one irreversible oxidation and one irreversible reduction for each compound as measured in N, N-dimethylformamide. Oxidation ranges from 0.94 V for III to 1.04 V for I and have been attributed to the Re-I/Re-II couple. In contrast, the reductions are ligand centered, ranging from -1.64 V for II to -1.90 V for III and V. Density functional theory calculations on the vertical one electron oxidized and one electron reduced species, using the gas-phase optimized geometry for the neutral complex confirm this assignment. Compounds I-V show two absorption bands, one around 410 nm (metal-to-ligand charge transfer (MLCT), Re-d pi -> pi*) and the other at similar to 300 nm (intraligand, pi -> pi*). Excitation at 400 nm at 77 K leads to unstructured and monoexponential emission with large Stokes shift, whose maxima vary between 570 (III) and 636 (II) nm. The quantum yields for these emissions in solution are intensified strongly going from air to argon equilibrated solution. Singlet oxygen quantum yields change from 0.03 (III) to 0.21 (IV). These data are consistent with emission from (MLCT)-M-3. The emission undergoes a bathochromic shift when R-1 is a pi-donating group (Cl or N-pyrazolyl) and a hypsochromic shift for a pi-acceptor (COOH). The bimolecular emission quenching rate constant by triethylamine (TEA) for II, IV, and V is 1.09, 0.745, and 0.583 x 10(8) M-1 s(-1), respectively. Photolysis in dichloromethane-CO2 saturated solution with TEA as a sacrificial electron donor leads in all cases to formic acid generation.

This study provides a phenomenological method to describe the exponential growth, saturation, and decay of coronavirus disease 2019 (COVID-19) deaths and infections via a Monte Carlo approach. The calculations connect Gompertz-type trial distributions of infected people per day with the distribution of deaths adopting two gamma distributions to account for the elapsed time that encompass the incubation and symptom onset to death periods. The analyses include death data from the USA, Brazil, Mexico, the United Kingdom (UK), India, and Russia, which comprise the four countries with the highest number of deaths and the four countries with the highest number of confirmed cases, as of August 07, 2020, according to theWorld Health Organization webpage. The Gompertz functions were fitted to the data of weekly averaged confirmed deaths per day by mapping the chi(2) values. The uncertainties, variances, and covariances of the model parameters were calculated by propagation, taking into account the standard errors of the data for each epidemiological week. The fitted functions for the average deaths per day for the USA and India have an upward trend, with the former having a higher growth rate and quite huge uncertainties. For Mexico, the UK, and Russia, the fits are consistent with a downward-sloping pattern. For Brazil we found a subtle trend down but with significant uncertainties. The USA, UK, and India data showed first peaks with higher growth rates compared with the second ones (4.2, 2.2, and 3.5 times higher, respectively), demonstrating the benefits of nonpharmacological interventions of sanitary measures and social distance flattening the secondary peaks of the pandemic. For the case of the USA, however, a third peak seems quite plausible, most likely related with the recent relaxation policies. Brazil’s data are satisfactorily described by two highly overlapped Gompertz functions with similar growth rates, suggesting a two-step process for the pandemic spreading. For the cases of Mexico and Russia single peaks with smoother slopes fitted the data satisfactorily. The 95% confidence intervals for the total number of deaths (x10(3)) predicted by the model for August 31, 2020, are 160 to 220, 110 to 130, 59 to 62, 41.3 to 41.4, 54 to 63, and 16.0 to 16.7 for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. Our estimates for the point prevalences of infections are compared with some preliminary data from serological studies and/or model calculations focused on the USA, Brazil, and UK scenarios. The point prevalences and 95% confidence intervals for August 1, 2020, were estimated to be 5.7 (3.9-7.5)%, 8.9 (7.4-10.3)%, 9.3 (8.3-10.3)%, 5.7 (4.5-6.9)%, 0.9 (0.8-1.0%), and 1.2 (1.0-1.3)% for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. The method represents an effective few-parameter framework to estimate the line shape of the infection curves and the uncertainties of the relevant parameters based on the actual death data of a pandemic.

Two new coordination compounds viz. [Ni(2,6-PDC)(Hdmpz)(H2O)(2)]center dot H2O (1) and [Zn(3-CNpy)(2)Cl-2] (2) (2,6-PDC = 2,6-pyridinedicarboxylate, Hdmpz = 3,5-dimethylpyrazole, 3-CNpy = 3-cyanopyridine) have been synthesized and characterized using elemental analysis, thermogravimetric analysis, electronic, infrared spectroscopy and single crystal X-ray diffraction techniques. Crystal structure analyses reveal the presence of supramolecular assemblies involving interesting dimers with unconventional contacts in the compounds. DFT (Density Functional Theory) calculations on the supramolecular dimers in the crystal structure of 1 reveal that the sum of contributions of anion-pi, pi-pi and other long range interactions due to the approximation of the bulk monomers is energetically significant. Molecular Electrostatic Potential (MEP) surface and Quantum Theory of Atoms in Molecules (QTAIM) analyses on the interesting supramolecular dimers of the crystal structures of 2 reveal the presence of unconventional anion center dot center dot center dot pi contacts involving coordinated chlorido ligands and C-H center dot center dot center dot pi(nitrile) interactions involving the pi-system of the nitrile moiety of 3-cyanopyridine. Remarkably, Atoms in Molecules analysis also confirms the existence of energetically significant unconventional anti-parallel nitrile center dot center dot center dot nitrile interaction in the crystal structure of 2. Cell cytotoxicity of the compounds performed in Dalton’s lymphoma (DL) malignant cancer cell line showed effective potency with negligible cytotoxicity in normal cells (similar to 12%). Direct N-glycosylation between glycals and amides/amines was achieved with exclusive stereoselectivity in moderate to high yields. The limitations of arene C-H functionalization of aryl sulfonamides containing strongly coordinating Nheterocycles were overcome using a Rh(III) catalyst. Purpose Sepsis survivors have a higher risk of rehospitalisation and of long-term mortality. We assessed the rate, diagnosis, and independent predictors for rehospitalisation in adult sepsis survivors. Methods We searched for non-randomized studies and randomized clinical trials in MEDLINE, Cochrane Library, Web of Science, and EMBASE (OVID interface, 1992-October 2019). The search strategy used controlled vocabulary terms and text words for sepsis and hospital readmission, limited to humans, and English language. Two authors independently selected studies and extracted data using predefined criteria and data extraction forms. Results The literature search identified 12,544 records. Among 56 studies (36 full and 20 conference abstracts) that met our inclusion criteria, all were non-randomised studies. Studies most often report 30-day rehospitalisation rate (mean 21.4%, 95% confidence interval [CI] 17.6-25.4%; N = 36 studies reporting 6,729,617 patients). The mean (95%CI) rehospitalisation rates increased from 9.3% (8.3-10.3%) by 7 days to 39.0% (22.0-59.4%) by 365 days. Infection was the most common rehospitalisation diagnosis. Risk factors that increased the rehospitalisation risk in sepsis survivors were generic characteristics such as older age, male, comorbidities, non-elective admissions, hospitalisation prior to index sepsis admission, and sepsis characteristics such as infection and illness severity, with hospital characteristics showing inconsistent associations. The overall certainty of evidence was moderate for rehospitalisation rates and low for risk factors. Conclusions Rehospitalisation events are common in sepsis survivors, with one in five rehospitalisation events occurring within 30 days of hospital discharge following an index sepsis admission. The generic and sepsis-specific characteristics at index sepsis admission are commonly reported risk factors for rehospitalisation. Registration PROSPERO CRD 42016039257, registered on 14-06-2016.

Self-assembly reaction of copper(II) ions and triple set of ligands (phenylgermaniumsesquioxane, 2,2′-bipyridine, 3,5-dimethylpyrazolate) results in the formation of the first example of tris-heteroleptic copper cage product (PhGeO2)(10)Cu-6(2,2′-bipy)(2)(3,5-Me(2)Pz)(2) (1). The ligation styles of complex’ components are different and includes (i) O-coordination from two cyclic pentamembered germsesquioxanes, (ii) N-coordination from two deprotonated 3,5-dimethylpyrazoles, (iii) N-ligation from two 2,2′-bipyridines. These features as well as other details of structure of 1 were established by X-day diffraction study. Analysis of the regioselectivity parameters found for the oxidation of linear and branched alkanes led to a conclusion that the reaction mechanism includes the formation of HO center dot radicals. However, the kinetic peculiarities of the cyclohexane oxidation with H2O2 in acetonitrile allowed to assume that the oxidation proceeds predominantly in a cavity generated inside of the tris-heteroleptic copper cage but not in the solution volume. Two coordination complexes of Cu(II) and Co(II) involving 3,5-dimethylpyrazole viz. { [Cu(mu-O,O’-SO4) (Hdmpz)(2)(H2O)]center dot 2H(2)O}(n) (1) and [Co(Hdmpz)(4)(H2O)(2)Cl-2 (2) (where Hdmpz = 3,5-dimethylpyrazole) have been synthesized in purely aqueous medium at room temperature. Crystal structure of complex 1 consists of a polymeric chain of distorted square pyramidal Cu(II) centers bridged by bidentate sulfato ligands. The polymeric chain in 1 has been stabilized by N-H center dot center dot center dot O and C-H center dot center dot center dot pi interactions. Particularly relevant is the polymeric chain in 1, since in addition to the afore mentioned non-covalent contacts, unprecedented chalcogen bonding interactions involving the sigma-hole at the tetrahedral sulfur atom are observed and characterized using theoretical calculations. To our knowledge, sigma-hole chalcogen bonding interactions involving tetrahedral sulfur atom have not been previously reported in the literature. The lattice water molecules self assembles the polymeric chains of 1 into a 3D supramolecular architecture via C-H center dot center dot center dot O hydrogen bonding interactions. The cationic units in the crystal structure of 2 self assembles via O-H center dot center dot center dot Cl, N-H center dot center dot center dot Cl and C-H center dot center dot center dot Cl weak hydrogen bonding interactions into a 2D supramolecular architecture induced by chloride anions in the lattice. PXRD patterns of 1 and 2 demonstrate good phase purity of the complexes. In addition, the thermal behaviour of the complexes has also been investigated.
