Purpose: Regorafenib is an oral multikinase inhibitor approved for the therapy of previously treated metastatic colorectal carcinoma (mCRC). The aim of the present study was to analyze the outcomes of treatment with regorafenib in real-world clinical practice based on data from a national registry. Methods: The CORECT registry, the Czech non-interventional database of patients with mCRC treated with targeted agents, searched for patients with metastatic CRC treated with regorafenib. In total, 555 evaluable patients were identified. Results: The median age at diagnosis was 61.7 years. All patients had disease progression on or after previous systemic treatment. Most patients were treated with an initial dose of 160 mg daily (n = 463; 83.6%). The median duration of treatment was 2.7 months (range 0.023.4 months). By the data cut-off date, 472 patients (85%) had completed treatment with regorafenib and were evaluable for treatment response evaluation. Partial response was reported in 13 patients (2.8%) and disease stabilization in 130 patients (27.5%). Median progression-free survival (PFS) and overall survival (OS) were 3.5 months (95% confidence interval [CI] 3.2-3.7 months) and 9.3 months (95% CI 8.3-10.3 months), respectively. The 6-month OS rate was 67.7% (95% CI 63.4-72.1%). Multivariable analysis showed that female gender, longer interval from diagnosis of metastatic disease, MO stage at diagnosis, and Eastern Cooperative Oncology Group performance status (ECOG PS) 0 were associated with longer PFS, while higher body-mass index (BMI), longer interval from diagnosis of metastatic disease, and ECOG PS of 0 were associated with longer OS. Conclusion: OS of patients treated with regorafenib in the real-world clinical practice in this cohort exceeded that reported in randomized trials. Regorafenib is a safe and active treatment option for a subgroup of patients with mCRC who are progressing after other systemic therapies and maintain good performance status.

Deregulation of the transcriptional repressor BCL6 enables tumorigenesis of germinal center B-cells, and hence BCL6 has been proposed as a therapeutic target for the treatment of diffuse large B-cell lymphoma (DLBCL). Herein we report the discovery of a series of benzimidazolone inhibitors of the protein-protein interaction between BCL6 and its co-repressors. Objective. Exposure to severe stressors may alter immune function and augment inflammation and cytokine release, increasing risk of autoimmune disease. We examined whether childhood abuse was associated with a heightened risk of incident systemic lupus erythematosus (SLE). Methods. Data were drawn from the Nurses’ Health Study II, a cohort of US female nurses enrolled in 1989, followed with biennial questionnaires. We measured childhood physical and emotional abuse with the Physical and Emotional Abuse Subscale of the Childhood Trauma Questionnaire and sexual abuse with the Sexual Maltreatment Scale of the Parent-Child Conflict Tactics Scale, both administered in 2001. We identified incident SLE (>= 4 American College of Rheumatology 1997 classification criteria) through 2015. We used multivariable Cox regression models to evaluate the association between childhood abuse and SLE, accounting for potential confounders (e.g., parental education, occupation, home ownership) and mediators [e.g., depression, posttraumatic stress disorder (PTSD)]. Results. Among 67,516 women, there were 94 cases of incident SLE. In adjusted models, exposure to the highest versus lowest physical and emotional abuse was associated with 2.57 times greater risk of SLE (95% CI 1.30-5.12). We found that 17% (p < 0.0001) of SLE risk associated with abuse could be explained by depression and 23% (p < 0.0001) by PTSD. We did not observe a statistically significant association with sexual abuse (HR 0.84, 95% CI 0.40-1.77, highest vs lowest exposure). Conclusion. We observed significantly increased risk of SLE among women who had experienced childhood physical and emotional abuse compared with women who had not. Four new derivatives of the versatile polyfunctional ligands 3-(pyrazol-1-yl)propanamide (1, = PPA) and 3-(3,5-dimethylpyrazol-1-yl)-propanamide (2, = Me(2)PPA) have been prepared, in which one of the amide hydrogens on 1 and 2 is formally replaced by alkyl residues. X-Ray diffraction studies revealed that, in contrast to 1 and 2, the N-isopropyl substituted derivatives 3 and 4 form supramolecular hydrogen-bonded chains rather than two-dimensional arrays, and the N-(2-methyl-4-oxypentan-2-yl) (=MOP) substituted compounds 5 and 6 exist as cyclic dimers in the solid state. Reactions of PdCl2(COD) (COD = 1,5-cyclooctadiene) with 2 equiv. of the corresponding ligands 1-6 in all cases afforded trans-PdCl2(L)(2) complexes (7-12). 3-Aminothieno[2,3-b]pyridine-2-carboxylic acid esters readily reacted with 3,5-dimethyl-1-(cyanoacetyl)-1H-pyrazole to give previously unknown N-(thieno[2,3-b]pyridin-3-yl)cyanoacetamides. Reactions of the latter with 2-(arylmethylidene)malononitriles were nonselective, and mixtures of different heterocyclization products were generally formed. The cyclization of ethyl 4,6-dimethyl-3-[(cyanoacetyl)amino]thieno[2,3-b]pyridine-2-carboxylate afforded 2,4-dihydroxy-7,9-dimethylthieno[2,3-b : 4,5-b’]dipyridine-3-carbonitrile whose tautomeric equilibrium was studied by DFT quantum chemical calculations. In silico analysis of biological activity of the synthesized compounds was performed.

Background: The Islamic Republic of Iran is at high risk of natural disasters. Households are the basic unit of society and they need to be prepared for disasters. Aims: This study assessed disaster preparedness among Iranian households in 2015. Methods: Using multistage cluster sampling, 2968 households in the catchment area of 53 public health departments in all 30 provinces of the Islamic Republic of Iran were surveyed on the disaster preparedness measures they had taken in the past year. The data collection questionnaire included 15 disaster preparedness measures. The mean preparedness score was calculated and its association with residence (urban versus rural), house ownership (owner versus renter) and education level of the head of the household (illiterate, elementary school, middle school, high school, university) was evaluated. Results: The mean score for household disaster preparedness was 9.3 out of 100 (95% confidence interval: 8.3-10.3). Multivariate analysis showed higher preparedness among rural than urban households (P = 0.02). Higher education level was positively associated with a higher preparedness score (P < 0.001), as was house ownership (P < 0.001). Conclusion: The level of household disaster preparedness in the Islamic Republic of Iran in 2015 was low. In line with the first priority for action of the Sendai Framework for Disaster Risk Reduction, the current study provides an overall picture of household disaster preparedness in the Islamic Republic of Iran. The first Pd-catalyzed asymmetric allenylic [4+1] cycloaddition was successfully developed. Alternatively, tuning the Pd catalyst switched the reactivity toward an unprecedented [4+3] cycloaddition/cross-coupling. Ligands play a vital role in controlling the reaction pathway, allowing highly selective access to different products from identical substrates. Biological evaluation of the obtained compounds led to the discovery of new antitumor targets. A possible mechanism is proposed, suggesting two interesting catalytic cycles for the cycloaddition with palladium-butadienyls. This study also demonstrated the potential and utility of allenic esters as 1,4-biselectrophiles and C-4 synthons for participating in cycloaddition reactions.

An enantioselective Mannich reaction between 3-fluorooxindoles and pyrazolinone ketimines has been developed for the construction of amino-pyrazolone-oxindoles containing stereogenic C-F units. Based on this new protocol that allows for the generation of two adjacent tetrasubstituted stereocenters, a variety of structurally diverse fluorinated amino-pyrazolone-oxindoles were obtained in good to excellent yields with excellent diastereoselectivities and enantioselectivities (up to 98% yield, >20 : 1 dr and >99% ee). What’s more, good yield and high stereoselectivities were obtained in the gram-scale reaction.

The changes in spectra and intensities of the streamwise component of turbulent velocity are calculated in the inflow of a turbine rotor. The flow is initially calculated in the limit when the turbulence is of small scale compared with the rotor diameter. Rapid distortion theory (RDT), Batchelor & Proudman (Q. J. Mech. Appl. Maths, vol. 7 (1), 1954, pp. 83-103) (BP), for small-scale turbulence is combined with the effect of the fluctuating potential flow field on the turbulence caused by the direct interaction of the incident turbulence with the rotor as a sheet of resistance. A second computation is then carried out for turbulence of larger length scale. The results of the calculations are compared with velocity measurements in the inflow of both a commercial wind turbine and a tidal turbine rotor.
